Randomised placebo-controlled trials (RPCTs) are the gold standard for evaluating novel treatments. However, this design is rarely used in the context of orthopaedic interventions where participants are assigned to a real or placebo surgery. The present study examines attitudes towards RPCTs for orthopaedic surgery among 687 orthopaedic surgeons across the USA. When presented with a vignette describing an RPCT for orthopaedic surgery, 52.3% of participants viewed it as ‘completely’ or ‘mostly’ unethical. Participants were also asked to rank-order the value of (...) five different types of evidence supporting the efficacy of a surgery, ranging from RPCT to an anecdotal report. Responses regarding RPCTs were polarised with 26.4% viewing it as theleastvaluable (even less valuable than an anecdote) and 35.7.% viewing it as themostvaluable. Where equipoise exists, if we want to subject orthopaedic surgeries to the highest standard of evidence (RPCTs) before they are implemented in clinical practice, it will be necessary to educate physicians on the value and ethics of placebo surgery control conditions. Otherwise, invasive procedures may be performed without any benefits beyond possible placebo effects. (shrink)

After a brief, personal reflection on Aron Gurwitsch's life and his many influences on my career, I devote this lecture to some of the central themes of a phenomenology of medicine. Its core is the clinical encounter, which displays a certain structure I term the asymmetry of power and vulnerability —a complex contextual imbalance characterized by multiple points of view, hence points for reflective entrance. These are then interpreted phenomenologically in terms of epoché and reduction, evidence, reflection, and other related (...) themes. This article introduces a satellite technology, High Resolution Picture Transmission (HRPT), for a science activity. HRPT is a digital data stream transmitted from environmental satellites. The HRPT imagery is capable of providing real-time studies of regional geology, glacial processes, atmospheric processes, ocean circulation, coastal processes, hydrology, and so on. Obtaining and manipulating HRPT data creates a variety of exciting experiences in science classrooms. A Science and Technology for Individuals, Societies, and the Environment (STISE) teaching model was developed to use satellite (...) technologies for science activities. STISE is a conceptual model for addressing key cognitive and affective parameters when designing science lessons. STISE is designed to encourage students to understand science and technology from their levels of cognition, then relate their understanding to the environment, levels of societies, and their lives. Relationships among science, technology, and society become both obvious and interesting to students as they process and interpret the HRPT imagery. (shrink)

This study attempts a systematic philosophical examination of C. G. Jung's understanding of the unconscious and, more particularly, of his understanding of das Selbst . Chapter 1 brings into focus the historical context of Jung's discussion by briefly examining the understanding of the unconscious in the work of four leading figures in late 19th century psychology: Wilhelm Wundt, Pierre Janet, Theodore Flournoy, and Sigmund Freud. Chapters 2 through 5 trace the development of Jung's thinking on the nature of the unconscious (...) and on the self; a close textual analysis is made of his writings from the earliest lectures, the Zofingia lectures , to his last major work, Mysterium Conjunctionis . ;Under philosophical scrutiny, Jung's theoretical positions are revealed to be somewhat inconsistent or simply inadequate. Jung does not offer to philosophy a rigorous, carefully worked out theoretical reflection on the nature and structure of the human being. Yet, this said, it is also true that Jung does have something to say on this issue, and it is the effort of this study to give theoretical coherence to Jung's reflections. ;Thus, as re-constructed, Jung's position may be stated as follows: The unconscious is an intelligent, transpersonal structure irreducible to consciousness. As an intelligent structure, the unconscious maintains identity-in-difference, and for this reason, Jung suggests, unconscious intellectual processes are irreducible to conscious intellectual processes which "discriminate." The supra-intelligent, transpersonal unconscious structure is a subject; the self is the supra-intelligent, transpersonal unconscious subject. While the self, the unconscious subject, is irreducible to the conscious subject , the conscious subject remains, nevertheless, a manifestation of the self. The ego and the self are mutually dependent or cor-related. ;In the conclusion of this study, I turn to the thought of Martin Heidegger to help elucidate the concerns of Jung in a more rigorously philosophical manner. The measurement of sensory intensity has had a long history, attracting the attention of investigators from many disciplines including physiology, psychology, physics, mathematics, philosophy, and even chemistry. While there has been a continuing doubt by some that sensation has the properties necessary for measurement, experiments designed to obtain estimates of sensory intensity have found that a general rule applies: Equal stimulus ratios produce equal sensory ratios. Theories concerning the basis for this simple psychophysical rule are discussed, with emphasis given to (...) the physical correlate theory, which considers judgments of subjective magnitudes to be based upon estimates of physical dimensions that vary regularly with changes in degree of stimulation. For the most thoroughly investigated sensory scales, brightness and loudness, the physical correlate is considered to be distance. Our “tacit knowledge” of the sensory effects of changing distance plays an essential role in matching motor activities to environmental conditions and in ensuring accurate perceptual evaluations through brightness and loudness constancies. In psychophysical experiments, subjects apparently use this same tacit knowledge when required to estimate relative subjective magnitudes. The evidence related to the physical correlate theory is summarized, and it is concluded that, while under appropriate conditions we demonstrate considerable skill in evaluating environmental relationships, we are quite unable to estimate the neurophysiological nature or quantity of sensory response. A psychophysics devoted to studying conditions required for accuracy and conditions producing errors in the perception of environmental relationships would seem to be more valuable than one devoted to subjective magnitudes. This book offers a powerful interpretation of the philosophy of William James. It focuses on the multiple directions in which James's philosophy moves and the inevitable contradictions that arise as a result. The first part of the book explores a range of James's doctrines in which he refuses to privilege any particular perspective: ethics, belief, free will, truth and meaning. The second part of the book turns to those doctrines where James privileges the perspective of mystical experience. Richard Gale (...) then shows how the relativistic tendencies can be reconciled with James's account of mystical experience. Motilal Shastri developed an ethical theory which closely resembles rule utilitarianism at roughly the same time as and yet in complete independence of English-speaking philosophers. The philosophic significance of his view lies in the manner in which he develops and justifies his position. Shastri contends that efficiency in action requires indifference or inattention to ends. He appears to use the same device for justifying rule-governed duties that Mill uses to justify a move from egoism to altruism: that actions first viewed (...) as means may later become ends in themselves. However, in Shastri’s theory, ends appear to be retained as unconscious motives. (shrink)

On the other side of the ledger there seem to be equally powerful reasons for countenancing negative events and states. The first and foremost reason is that since every true proposition about the world supposedly corresponds with some event, there must be a negative event corresponding to every true negative proposition about the world. Second, there can be no determinate reality without negative facts, since something can be a definite individual only if it has finite boundaries, which require that it (...) is not the same as everything else. Third, change requires temporal passage consisting in the shift of the present to ever later times; however, this requires that there be a past and future, which are the no-longer and the not-yet. Fourth, we seem to perceive negative things such as lacks and absences, but whatever we perceive must exist. Fifth, for those who believe in real essences or substantial natures, there are objective teleological negations consisting in a certain kind of object not realizing all of its natural functions. And, finally, it seems to be inconsistent to say that there are no negative facts, for supposedly there will be at least one negative fact consisting in there not being any negative facts. (shrink)
